You cannot shake off a sudden urge to indulge in a bar of chocolate despite having just eaten lunch; or perhaps it is the frequent yawning even after a good night\u2019s sleep.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">The day after, it strikes \u2013 one of those crippling headaches that can last up to three days<sup> <\/sup>and leave you feeling drained, nauseated and needing to recuperate in a quiet, dark room.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">These sudden food cravings or frequent yawning could be more than isolated incidents. According to the Mayo Clinic, these and other warning signs such as a stiff neck<sup>1<\/sup>, unusual tiredness or even unexpected sadness could mean that a migraine episode is just one or two days away<sup>1<\/sup>.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">It may be common practice to pop a pill and push through the pain, but this may not prevent future migraines \u2013 and over time, it could even make things worse<sup>1<\/sup>.<\/p>\n<h2 class=\"font-primary-headline-md-semibold\" data-testid=\"article-subhead-test-id\">What\u2019s the difference between headache and migraine?<\/h2>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">According to Adjunct Associate Professor Jonathan Ong, senior consultant, Division of Neurology, Department of Medicine, National University Hospital (NUH), the above could be signs of something more than a common headache. Prof Ong is also the current president of the <a href=\"https:\/\/headachesocietysingapore.com\/?ref=inline-article\" rel=\"nofollow noopener\" class=\"gap-x-04 items-center inline text-primary-60 select-auto\" aria-label=\"link\" target=\"_blank\" data-testid=\"custom-link\"><span class=\"inline font-tertiary-body-baseline-regular\" data-testid=\"typography-test-id\"><u>Headache Society of Singapore<\/u><\/span><\/a>.<\/p>\n<p>He explains: \u201cA headache is a symptom, not a diagnosis<sup>2<\/sup>. Because migraine is a neurological condition affecting multiple brain pathways<sup>3<\/sup>, it can sometimes occur without significant head pain, but with other symptoms.\u201d\u00a0<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">\u201cJust as an engine warning light on a car dashboard signals that something is wrong but doesn\u2019t identify the cause, a symptom signals that there is a problem, while a diagnosis identifies the underlying cause \u2013 such as a faulty spark plug.\u201d<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Migraine commonly causes recurrent attacks\u00b2 characterised by pulsating pain, sensitivity to movement, light and sound, as well as nausea and vomiting<sup>1,2,4<\/sup>.\u00a0<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Other key signs of migraine<sup>2<\/sup> may include intense, often one-sided throbbing pain, visual or neurological disturbances, such as flashing lights, jagged zigzag lines, blurry patches or blind spots<sup>1<\/sup>, collectively known as auras<sup>2<\/sup>.<\/p>\n<h2 class=\"font-primary-headline-md-semibold\" data-testid=\"article-subhead-test-id\">How common is migraine in Singapore?<\/h2>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">In Singapore, <a href=\"https:\/\/urbanliving.com.sg\/health\/general\/migraine-treatment-singapore-causes-medications-cost\/?ref=inline-article\" rel=\"nofollow noopener\" class=\"gap-x-04 items-center inline text-primary-60 select-auto\" aria-label=\"link\" target=\"_blank\" data-testid=\"custom-link\"><span class=\"inline font-tertiary-body-baseline-regular\" data-testid=\"typography-test-id\"><u>roughly 1 in 10 adults \u2013 or around 330,000 people<\/u><\/span><\/a> \u2013 are affected by migraine<sup>5<\/sup>, with the condition up to three times more common in women after puberty due to hormonal factors such as menstrual cycle changes, contraception or hormonal therapy<sup>6<\/sup>.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">A local study, <a href=\"https:\/\/journals.sagepub.com\/doi\/abs\/10.1177\/2515816320908241?ref=inline-article\" rel=\"nofollow noopener\" class=\"gap-x-04 items-center inline text-primary-60 select-auto\" aria-label=\"link\" target=\"_blank\" data-testid=\"custom-link\"><span class=\"inline font-tertiary-body-baseline-regular\" data-testid=\"typography-test-id\"><u>Economic Burden of Migraine in Singapore<\/u><\/span><\/a>, estimated that migraine resulted in <a href=\"https:\/\/www.straitstimes.com\/singapore\/health\/migraine-sufferers-cost-singapore-104-billion-in-economic-losses-last-year?ref=inline-article\" rel=\"nofollow noopener\" class=\"gap-x-04 items-center inline text-primary-60 select-auto\" aria-label=\"link\" target=\"_blank\" data-testid=\"custom-link\"><span class=\"inline font-tertiary-body-baseline-regular\" data-testid=\"typography-test-id\"><u>$1.04 billion in economic losses<\/u><\/span><\/a> in 2018<sup>5,7<\/sup>. This was mostly due to lost productivity, and the remaining 20 per cent was attributed to healthcare costs, such as medical tests and consultations. There is a need to raise awareness among both healthcare professionals and the general public, and significant opportunities remain for further improvement.\u201d<\/p>\n<h2 class=\"font-primary-headline-md-semibold\" data-testid=\"article-subhead-test-id\">Episodic vs chronic migraine: when to seek treatment<\/h2>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Even among people with migraine, there is a distinction between those who suffer from episodic and chronic migraines<sup>10<\/sup>.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Prof Ong explains that the latter, which he says affects about 2.5 per cent<sup>10 <\/sup>of the entire migraine cohort, means a person experiences migraine headaches on 15 or more days each month<sup>10<\/sup>,<sup> <\/sup>for more than three months.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">\u201cChronic migraine often develops from episodic migraine when symptoms such as headache and nausea aren\u2019t managed well<sup>11<\/sup>,\u201d says Prof Ong.<\/p>\n<p>That is where preventive care is key, he adds. \u201cEarly recognition and appropriate management may help reduce the risk of episodic migraine progressing to chronic migraine<sup>11<\/sup>.\u201d<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">For people whose episodic migraine is starting to disrupt their daily life, a combination of preventive treatment<sup>4 <\/sup>and lifestyle adjustments<sup>4<\/sup> may be recommended. For chronic migraine, preventive treatment is usually necessary to reduce both frequency and severity<sup>12, 13<\/sup>.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Yet in reality, however, it is not so simple, notes Prof Ong. \u201cMigraine can be highly disabling<sup>14<\/sup>. Keeping a daily diary<sup>15 <\/sup>can help identify these triggers, says Prof Ong.\u00a0<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Stress is one of the most commonly reported migraine triggers, affecting around 70 per cent of people with migraine.<sup>16<\/sup> Some people may also experience the \u201clet-down\u201d effect, where an attack occurs after a high-stress period<sup>17<\/sup>.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">\u201cThe migraine brain thrives on routine,\u201d he says. \u201cEven taking a break from work can trigger what\u2019s known as a \u2018holiday headache\u2019. People often assume less stress means fewer migraines, but it\u2019s the disruption to routine<sup>18<\/sup> that can trigger an attack.\u201d<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">\u201cSleeping in, changing meal times, travelling or crossing time zones<sup>18,19<\/sup> can all be enough to trigger symptoms in some people.\u201d<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Lifestyle and environmental factors can also trigger migraine, including dehydration, skipping meals, caffeine and abrupt temperature changes<sup>1,19<\/sup>, such as moving from a warm environment into an air-conditioned room.\u00a0<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Frequent use of pain-relieving medicines over time may contribute to medication overuse headache<sup>20 <\/sup>in some people.\u00a0<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Research has helped doctors better understand what happens in the brain during a migraine attack. One area involved is the hypothalamus, which helps regulate functions such as sleep, appetite and hormones<sup>3<\/sup>.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Prof Ong explains: \u201cResearchers have identified CGRP, or calcitonin gene-related peptide, as one of the key molecules involved in migraine<sup>4<\/sup>. Newer treatments either target CGRP or block its receptor<sup>4<\/sup>, helping to interrupt a pathway involved in migraine\u2074.\u201d<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Because these therapies are more targeted, he says, they are designed to minimise effects on other pathways, which may improve tolerability and safety<sup>4<\/sup>.\u00a0<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">He says: \u201cMigraine treatment is no longer a one-size-fits-all approach.
