{"id":71880,"date":"2026-07-26T22:47:01","date_gmt":"2026-07-26T14:47:01","guid":{"rendered":"https:\/\/sgbuzz.com\/?p=71880"},"modified":"2026-07-26T22:47:01","modified_gmt":"2026-07-26T14:47:01","slug":"imh-nurses-code-grey-initiative-reduces-assaults","status":"publish","type":"post","link":"https:\/\/sgbuzz.com\/?p=71880","title":{"rendered":"IMH nurses&#8217; Code Grey initiative reduces assaults"},"content":{"rendered":"<p><br \/>\n<\/p>\n<div>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">SINGAPORE \u2013 <!-- -->Something as simple as a different turn of phrase has the power to prevent a tense situation from turning violent.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">By specially training nurses in techniques ranging from de-escalation to negotiation to deal with incidents before they get out of hand, the Institute of Mental Health (IMH) was able to cut the number of physical assaults by half during a six-month trial.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">The institute now plans to scale up this effort, called the Code Grey team initiative, to all 40 of its wards by December 2027 to elevate psychiatric care standards and create safer workplaces for staff.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">In the healthcare setting, \u201cCode Blue\u201d refers to a life-threatening medical emergency. \u201cCode Grey\u201d, on the other hand, is a psychiatric emergency that requires a response appropriate to the \u201cgrey situation\u201d of dealing safely with somebody who is aggressive or disruptive, but unarmed.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">The IMH initiative was helmed by two IMH front-line staff \u2013 assistant director of nursing Kalaivanan Dakshnamoorthy, 47, and senior nurse manager Win Min Htet, 36.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">IMH staff care for patients with acute psychiatric conditions every day, which is meaningful work, though demanding, they said.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">When patients become acutely unwell, they may not be fully aware of their own conditions and can feel distressed. This can escalate rapidly to aggression.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">\u201cThere are various treatment modalities that nurses have to use, one of which is using themselves as a therapeutic tool to calm the patient down, using their verbal de-escalation skills. As long as the end result of the crisis doesn\u2019t result in the patient being restrained, that is successful de-escalation,\u201d Kalaivanan said.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">All IMH clinical staff go through a standard three-day Care and Response (C&amp;R) training programme, and have the skills and knowledge to manage disturbed patients. Code Grey takes this further by giving staff additional intensive training.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">IMH ran the six-month pilot from January to July 2025, where 20 registered nurses \u2013 those with a diploma or a bachelor\u2019s degree in nursing \u2013 were specially selected for Code Grey training, based on their personality, interpersonal skills and medical fitness.<\/p>\n<h2 class=\"font-primary-headline-md-semibold\" data-testid=\"article-subhead-test-id\">Three-day intensive training programme<\/h2>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">The three-day Code Grey training programme taught the nurses skills related to risk assessment, de-escalation, negotiation and physical intervention.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">For instance, Kalaivanan said that commonly used phrases such as \u201cWhat is your problem?\u201d or \u201cCalm down\u201d might end up triggering patients instead of resolving the issue at hand, as these might be perceived as blaming, dismissive or pressurising to the patients.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Examples of alternative phrases include \u201cHelp me understand what happened?\u201d or \u201cLet\u2019s take a moment to work through this together\u201d, he added.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Such phrases empower and assure patients they play a part in co-managing their own condition.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">\u201cNursing used to be paternalistic in nature, but now it is more about collaboration&#8230; We are really advocating independence for patients&#8230; we are moving towards person-centred care,\u201d Kalaivanan said.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Training included role-play and scenario training, some with people acting as patients, as well as dialogues with <!-- -->senior <!-- -->nursing leaders who spoke about their experiences.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Code Grey-trained nurses were better able to build rapport with at-risk patients, identify early warning signs of aggression such as clenching fists, staring into the eyes of staff and pacing around. I think sometimes we really can go in in a therapeutic manner. We are not against anybody,\u201d Kalaivanan said.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">During the pilot, Code Grey-trained nurses were deployed across four wards that handled more first-line cases, such as those admitted through the emergency department.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Incidents of physical assaults in these wards, such as staff being pushed, punched or slapped, were reduced by almost half, from 108 to 58 cases.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Staff also reported that they consistently felt safer, had much better support and felt more capable of dealing with tense situations.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">IMH said its patients also benefited from the calmer living environment.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">The plan to scale the initiative to about 40 wards by the end of 2027 will translate to five Code Grey-trained nurses in each ward to ensure there will be at least one or two on duty round the clock.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">About 40 per cent of IMH\u2019s 3,000 staff members are nurses, making them the largest staff group, ahead of doctors, allied health, ancillary and administrative staff.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Training for new batches of Code Grey nurses began in mid-June.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Kalaivanan and Win Min Htet spent more than three months from late 2024 developing the training curriculum from scratch.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Their research involved examining evidence in current literature as well as consolidating practical lessons from their experiences as C&amp;R instructors.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Their <!-- -->Code Grey <!-- -->project was recognised when IMH clinched the Wellbeing Innovation Award at the <a href=\"https:\/\/www.straitstimes.com\/singapore\/health\/moh-emphasises-mental-health-prevention-tackling-tech-impact-on-youth?ref=inline-article\" rel=\"noopener\" class=\"gap-x-04 items-center inline text-primary-60 select-auto\" aria-label=\"link\" target=\"_blank\" data-testid=\"custom-link\"><span class=\"inline font-tertiary-body-baseline-regular\" data-testid=\"typography-test-id\">WorkWell Leaders Award 2026<\/span><\/a> on April 24.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">The awards were presented by <!-- -->local <!-- -->charity WorkWell Leaders to recognise leadership that successfully advances organisational well-being.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Kalaivanan said the recognition was unexpected and that the duo had started the project as a way to protect both patients and colleagues.<\/p>\n<p class=\"text-primary font-tertiary-body-baseline-regular\" data-testid=\"article-paragraph-annotation-test-id\">Added Win Min Htet: \u201cAs staff at a psychiatric hospital, we really want to prioritise the well-being of our staff and also our patients\u2019 safety.\u201d<\/p>\n<\/div>\n<p><br \/>\n<center><br \/>\n<br \/><a href=\"https:\/\/www.straitstimes.com\/singapore\/health\/imh-nurses-trained-to-defuse-tense-situations-before-they-escalate-into-abuse-and-assault\" target=\"_blank\" rel=\"noopener\">Read Full Article At Source <\/a><br \/>\n<center\/><\/p>\n","protected":false},"excerpt":{"rendered":"<p>SINGAPORE \u2013 Something as simple as a different turn of phrase has the power to prevent a tense situation from turning violent.
