{"id":41728,"date":"2026-04-04T18:12:48","date_gmt":"2026-04-04T10:12:48","guid":{"rendered":"https:\/\/sgbuzz.com\/?p=41728"},"modified":"2026-04-04T18:12:48","modified_gmt":"2026-04-04T10:12:48","slug":"jogger-rushed-to-hospital-after-super-rare-allergic-reaction-triggered-by-exercise","status":"publish","type":"post","link":"https:\/\/sgbuzz.com\/?p=41728","title":{"rendered":"Jogger rushed to hospital after \u2018super rare\u2019 allergic reaction triggered by exercise"},"content":{"rendered":"<p><br \/>\n<\/p>\n<div>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">SINGAPORE \u2013<!-- --> Ms Selina Ang was set for her weekly Tuesday 10km run in the Marina Bay area with her colleagues, but along the way, she developed odd symptoms.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cI was around 7km in when I felt my tongue swelling. It felt weird, but I kept going as I was only 3km away from the end point,\u201d the associate director of marketing, communications and corporate sustainability told The Straits Times. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cHowever, I had to stop by the time I got to the 8.5km mark because the swelling got worse and began affecting my breathing.\u201d<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">The 50-year-old felt dizzy, suffered muscle spasms and abdominal cramps, and developed hives. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">She told her colleagues, who bundled her into a car and rushed her to the emergency department of Singapore General Hospital (SGH).<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">There, she was given an adrenaline jab to immediately reverse her symptoms, helping to open her airways and raise her blood pressure.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cAn ENT (ear, nose and throat) specialist was later called in to do a scope to ensure there was no more swelling in my throat and airways. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cI was put on several intravenous drips and was in the observation ward in case the symptoms flared up again despite the medications that were administered,\u201d Ms Ang said. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">She was sent home only at 1pm the next<!-- --> day,<!-- --> after the swelling in her mouth subsided. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">Her diagnosis: Exercise-induced anaphylaxis (EIAn).<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">Recalling the incident, which happened in February, Ms Ang said: \u201cThe senior doctor who saw me at the SGH emergency department told me that EIAn was \u2018super rare\u2019 and I was perhaps the first case she had ever seen.\u201d <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">Someone with this condition can get a severe and potentially life-threatening allergic reaction triggered purely by physical activity. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">EIAn is very rare<!-- -->, and while there are theories on what causes it and what the underlying immunological mechanism is, \u201cit is not well studied\u201d, said Dr Yeong Yao Qun, an associate consultant in emergency medicine at SGH.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cThe diagnosis of anaphylaxis is a clinical one, made based on possible allergen exposure. In the case of EIAn, the diagnosis of the \u2018exercise\u2019 portion is one of exclusion \u2013 when no other allergens are identified,\u201d he said. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">Treatment depends on the symptoms, which can include itchy skin rashes, swelling of the tongue, lips or throat, trouble breathing, and low blood pressure that may make a person feel dizzy, faint or weak, he added.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">After<!-- --> being treated for anaphylaxis and monitored for between four and six hours to ensure there is no recurrence of symptoms after the initial reaction, the patient is discharged and usually referred to an allergy specialist later to identify what might have triggered the reaction. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">While anaphylaxis can be induced purely by exercise, there is also food-dependent exercise-induced anaphylaxis.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">This, according to Associate Professor Lee Haur Yueh, a dermatologist at SGH, is more common, though also underdiagnosed.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">He said wheat products, nuts or shellfish, for example, when eaten by a patient who is already sensitive to them before exercise, might trigger a reaction. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">Prof Lee said: \u201cThe individual is typically able to tolerate the culprit foods and exercise on their own. Despite her exercise-induced anaphylaxis, she is glad her doctor did not advise her to stop exercising.<\/p>\n<p class=\"font-eyebrow-baseline-regular text-placeholder\" data-testid=\"inline-media-credit-test-id\">PHOTO: COURTESY OF SELINA ANG<\/p>\n<\/figcaption><\/figure>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">Ms Ang said the only food she reacts to is \u201cprawns that are not fresh\u201d.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cI get itchy patches after eating them,\u201d she said, admitting that she had eaten \u201ca couple of battered prawns for lunch before that fateful run in February\u201d. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cIt could have triggered my anaphylaxis,\u201d she said, although she was not diagnosed with food-dependent EIAn.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">Any patient suspected of having exercise-related anaphylaxis will need a comprehensive allergy evaluation. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cThis is a careful (review) and evaluation of possible food-dependent exercise-induced anaphylaxis, which is far more common,\u201d said Prof Lee.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cBlood tests and skin prick tests might identify food triggers, and these triggers should be avoided four to six hours prior to exercise.\u201d <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">These individuals would be prescribed an EpiPen, a handheld pen-like injection device used to deliver a shot of adrenaline during a severe allergic reaction. <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cThis should always be carried around,\u201d said Prof Lee. \u201cExercise should also be done with a companion after medical clearance.\u201d <\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">Ms Ang, who often goes on trekking holidays off the beaten track, has not let her condition stop her.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">She is currently in Nepal with her 18-year-old son Caleb Lim, \u201cconquering\u201d the Ghorepani Poon Hill trek \u2013 a moderate-level trek in the Annapurna region.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">\u201cWe are doing this mother-and-son trip before he does his national service,\u201d she said.<\/p>\n<p class=\"font-body-baseline-regular text-primary\" data-testid=\"article-paragraph-annotation-test-id\">For peace of mind, <!-- -->not only for herself but her son as well, <!-- -->she has two EpiPens in her backpack. <\/p>\n<figure class=\"portrait inline-media-wrapper\" style=\"--aspect-article-portrait:1140 \/ 1520\" data-testid=\"inline-media-test-id\">\n<div class=\"flex flex-col items-start relative w-fit\"><picture><source media=\"(max-width: 480px)\" srcset=\"https:\/\/cassette.sphdigital.com.sg\/image\/straitstimes\/2a195d154fee78384cec8ae5495e31886e8a3ca34d4e17dd3964cf01c3f4928a?w=480\"\/><source media=\"(min-width: 481px) and (max-width: 720px)\" srcset=\"https:\/\/cassette.sphdigital.com.sg\/image\/straitstimes\/2a195d154fee78384cec8ae5495e31886e8a3ca34d4e17dd3964cf01c3f4928a?w=720\"\/><source media=\"(min-width: 721px)\" srcset=\"https:\/\/cassette.sphdigital.com.sg\/image\/straitstimes\/2a195d154fee78384cec8ae5495e31886e8a3ca34d4e17dd3964cf01c3f4928a?w=900\"\/><img decoding=\"async\" src=\"https:\/\/cassette.sphdigital.com.sg\/image\/straitstimes\/2a195d154fee78384cec8ae5495e31886e8a3ca34d4e17dd3964cf01c3f4928a\" alt=\"\" class=\"aspect-portrait flex items-start shrink-0 portrait article-portrait object-contain mobile:w-auto tablet:w-auto\" data-testid=\"image-test-id\" loading=\"lazy\"\/><\/picture><\/div><figcaption class=\"mobile:mx-16 tablet:mx-00 flex flex-col gap-08 py-16 desktop:pb-24\">\n<p class=\"font-eyebrow-baseline-regular text-secondary\" data-testid=\"inline-media-caption-test-id\">Ms Selina Ang is currently in Nepal with her son Caleb Lim, 18.
